Background
The Carlton-Cook-Lake-St. Louis Community Health Board (CHB) was established through a Joint Powers Agreement in 1977. The Board is made up of nine members - 3 members from St. Louis County and 2 members from Carlton, Cook and Lake Counties. The Board meets 4 -5 times each year.

- Carlton County: Commissioner Marv Bodie and Barb Little;
- Cook County: Commissioner Janice Hall and Beverly Green;
- Lake County: Commissioner Tom Clifford, MD and Brad Alm;
- St. Louis County: Commissioner Steve O'Neil, Commissioner Mike Forsman
and Commissioner Chris Dahlberg;
Mission Statement
The mission of the Carlton-Cook-Lake-St. Louis Community Health Board is to work together to assess and address public health issues; strengthen local public health; provide fiscal and programmatic administrative oversight with the goal of increasing effectiveness.
Vision
The Community Health Board (CHB) will enhance collaborative efforts among member counties and community partners to strengthen public health in the region and the state to achieve optimal health for all.
